Can you remember what stores carried those mouse pads since it looks like most major chains such as Best Buy don't like carrying cloth pads anymore. Might have to order one online.

How much smaller? I recall a survey on UNorth and the average distance per 360 was about 22 cm or 8.6". That puts me at a quicker than average sens. and makes Kal slower than average. In more ways that one! http://www.instantrimshot.com/
Playing bots is pointless, and putting yourself through the torture that is playing Godlike bots is even pointlesser. They don't act like humans at all, so there's no need to go above Adept. Below Adept they won't use all routes in a map, and at that level there's a chance they'll miss from up close.
The problem, of course, is that whoever coded the bots' aim was bad at programming and did it completely wrong. Don't expect to ever be sniped below Godlike, and even then they're about 14x less effective than human players from a distance. D:

Are you referring to mouse OS settings because there I cannot turn off acceleration completely only slow it to a crawl. And the in-game mouse setting of .45--is this for sensitivity because it seems agonizingly slow?

I use 90 in UT2004 and 80 in CoD4 (the max). FOV is related to sensitivity -- well, not directly, but the feel of sensitivity is related to the distance it takes to turn 360, and FOV changes this -- so if you adjust one, you need to adjust the other by an equal amount. If you go from 90 to 100 FOV, you should adjust your mouse sensitivity down by 11% or so to offset the FOV change.
Vsync will do nothing but cause mouse lag -- leave it off and never, ever, ever turn it on.
For maximum smoothness, you'll probably want a steady framerate at least 90. I do 125 is CoD4. However smooth it'll be depends on your refresh rate too. In my case, 125 FPS @ 75 Hz feels smooth, whereas 90 FPS @ 100 Hz may not.
