It might sound strange, but sometimes imagines like this are deliberately created.
The general theory is that they're designed to make your brain subconsciously notice and focus on the picture. Mistakes, or things that "don't look right" are a good way to do this. The trick is making the image subtle enough not to be too noticeable as a "mistake". Too subtle, there's no "pickup". Too obvious, and it goes from being subconscious recognition to conscious, and the image looks noticeably weird.
The key is making a "mistake" that no-one will notice. If that's what they did with GTA4's artwork, then they did a good job. Put it this way, how many of you noticed it before it got pointed out to you?

The GTA chick actually has the correct number of fingers. The 4th and 5th digits labelled in the picture are the same one and it's just an optical illusion in the artwork.

The GTA chick picture actually doesn't exist, it's just an optical illusion in your monitor because there are multiple pixels of different colors. Seriously, the only person that knows if it's a sixth finger or not is the artist. If it isn't though then he put a weird arse shadow on the index finger, if it is well then it makes more sense, especially if there's another picture like that. (the driveby gun one posted)

Looks to me like he simply forgot to paint the straw that the lollipop is on ontop of the thumb. I imagine he had difficulty with it, painted the thumb ontop a few times and forgot to correct the lolli. Easy mistake
That said, bearing FlashIV's image in mind it's possible that the artist is simply doing it on purpose. He wouldn't be the first one to be playing a prank in his imagery.
