Team name - Every team has to at least have a name, even if it is just some random word or whatever. It'll allow people to at least refer to you by your team name. Do not attempt to mislead people in thinking that you own a registered company or otherwise.

Project name - What is your project's name, even if it is a temporary project name, it'll help you when people email or message you about it.

Brief description - Just a very brief description about your mod and what it is about. There's really no need to go into the story line to much, but at least something that will let people know what your mod is about and what it does. That way people interested in those particular details will be excited and will want to work on the mod. Also discuss how long the project has been in the works, and what sort of genre and play style you wish to go for. If that's not really clear yet, then perhaps stating the team mission or goals would be a good idea.

Compensation - Often mod teams don't offer any sort of cash payments. But if you do plan to change the mod into something else later on, perhaps you want to clear right from the get go.

Talent needed - A bullet point list of the types of talent you need, whether it is artist, technical artist, programmer, unrealscript programmer, web developer, level designer, user interface designer, etc. You need to list who you are looking for, otherwise people have no idea if they can apply for any positions. You should also list your expectations of what they will be doing, and if any experience is required (match this carefully with your own level of experience!)

Team structure - List who are already in your team and what they already do. Either user names or real names will suffice. This just allows people a chance to see who they may be working with. Most importantly, actually state what it is you do, and what is your role and position in the team.

Website - Does your mod have a website? If it does, then put it here so that people can find more information about your mod.

Contacts - List of contacts so that people can reach you or your team.

Previous Work by Team - List any sort of previous work you have done. This gives people a chance to see what sort of quality their own work needs to be, or at least place some faith into the project.

Additional Info - Any other additional information about your mod. This can range from concept art to screenshots.

Feedback - Sometimes you may wish to see some feedback, regarding the mod idea or something else. If you happen to use some body else's intellectual property, copy right or trade mark then perhaps here is a good place to clarify that you have asked permission from the respective owners.
