A few chests will be out in the open and easily seen but this isn't the complete picture.
Gearbox saw fit to put a lot of chests in completely out of the way places.
Always check your surroundings. Explore! Explore!
Most chests I've found were on top of buildings, or behind some structure which requires some inventive means of getting there. If you only find one chest during a mission, chances are you're not exploring enough. Dig around a bit, try jumping onto random objects and see where it takes you.
Don't be afraid of making a leap of faith.
Go ahead and jump to some random ledge as you never know what you'll find. I jumped from a cliff ledge, on a whim none the less, onto a small outcropping of a buildings roof. I followed it around and there was a chest all by itself in a small alcove. It was impossible to see from the ground. There was no way of knowing it was there unless someone randomly decided to jump onto that roof like I did.
In New Haven alone me and a friend have found 3 or 4 chests way up on top of buildings and completely hidden, some better than others.
Types of chests:
There are random small containers here and there. Mailboxes, small cash boxes, lockers, dumpsters, etc. These will always contain either ammo, cash, or lesser weapons, with the rare colored weapon.

After that is the large white square boxes. These always contain weapons or shields, but no ammo.

The last, and best chance of something good is the large red chest. These always contain weapons, shields, mods, and ammo. These large red chests are the ones that are hidden the most and require a bit of searching.
